List of conversion offsets/formulae between game versions

This page is intended as a reference for address offsets and formulae for converting codes between different versions of a game (1.0, 1.1 on the same platform, or 1.0 on one platform, 1.0 on another platform, etc), so as to provide a centralized list. Without further explanation, I shall add a few. Feel free to contribute.
